Practice Agreement Model
Defines any limitations for a PA to practice medicine based on their education, training, and experience.
Public Act 379 of 2016 requires every Physician Associate to have a practice agreement which must include a participating physician.
A practice agreement must include any limitations when a PA is providing medical treatment to a patient. Practice agreements can, and should, be tailored to meet the needs of each practice setting based on the PA's education, training and experience.
MAPA has designed this model practice agreement which includes everything required by law.*
